Founded in 1906, the agency has grown to serve nearly 11,000 individuals last year and is proud to be expanding their primary care / family practice service line in Greater Lafayette and select surrounding counties.
Valley Oaks Health is looking for an Associate Medical Director of Community Health to join our team and help foster an inclusive and vibrant community.
This position will play a critical leadership role in the development of Valley Oaks Health's medical practice model, ensuring integration of care across our continuum, leading continuous performance improvement efforts, developing professional relationships across community stakeholders, and be ultimately responsible for the delivery of superb medical services throughout each of our health clinics.
In addition to supervising medical service delivery via APRN's, this position is expected to provide direct care 50% of the time.
The successful candidate will have leadership experience in a multi-site medical practice (FQHC preferred), able to work with a variety of people to achieve practice goals, encourage bravery and accountability in the workplace, and demonstrate a desire to build a healthy, patient centered, regional health care system focus on improving access to integrated care for people who experience multiple disadvantages.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
