Emergency Department and Hospital consults and emergencies Common Diagnoses/Procedures:
Hydronephrosis with renal and ureteral calculous obstruction, Calculus of kidney, Benign prostatic , hyperplasia with lower urinary tract symptoms, Calculus of ureter, Urinary tract infection Patient volume:
0-10 daily Only 1 full time Urologist in FPN Need additional coverage for Hospital not for the office Only ED and Hospital work, no office Cover emergencies and any needing surgery Required:
Must Use EPIC Active IN license, DEA and CSR Clean file to start in June CV must be in Month / Year format Must have license in hand at time of submission, or proof that the license application has been submitted Any gaps greater than 30 days will require an explanation on the CV DEA must remain in state to maintain privileges.
Provider must be willing transfer and maintain DEA in state for the duration of privileges being active, or obtain a new in state DEA to remain in state for the duration of privileges being active Current Flu Vaccine Must be fully vaccinated for Covid 19 Exemptions only allowed for religious or medical reasons, once approved by the facility.
